Could be. Or, maybe the age of the man who fell (he was in his 80s) had something to do with it? He fell on a man and woman in their 60s, the gentleman also died but the woman thankfully sustained non-life-threatening injuries. Before anyone thinks ageism on my part, I’m 70 so…

These accidents usually end up as poor design of railing systems. But in defense it is difficult to design a railing that fits the entire population of body types and szes and still be able to see a show.
