Yes that’s what I did, too, after recognizing that there’s no option to search for terms or logical combinations here.
We know this is untrue as nearly all arrive in perfect condition when shipped to others.
Hence the mystery.
I t tried to power up the replacement P15 again today and the same issue. It doesn’t get past initializing and the home screen just shows five red receptacles. It just continues to say initializing across the top of the screen. I tried this with two different cables. The icons on the touchscreen are unresponsive. I sent photos to James and Aaron at PS Audio and I’ll see what they have to say. As I stated before, the packaging in both units seem to be excellent with no external damage noted.

I wonder about the SD card, inserted upside down perhaps, maybe that makes a difference? Unsure, not at home at the moment to check with mine.
Isn’t the purpose of the SD card to be used only when there is a firmware update for the P 15. It is not required for the unit to work properly. I did try inserting the SD card anyway to see if that made a difference.It inserted easily the correct way, and there was no change
Yes, but if it was inserted improperly the unit might look for software not there–idk, was just putting a possibility out there.
Had the same issue. An internal board replacement fixed the problem in a few minutes. Let PSA staff driving you to a solution.

Third time a charm. After receiving two P15s that failed to initialize I was prepared to move on. Emailed with James and decided to give it one more shot and the 3rd P15 delivered to me worked properly. I’ve had it in my system for two weeks, and I’m very pleased with the performance. I have very poor power up where I live with the incoming THD always over 6%. The P 15 has improved the clarity of my system and definitely blacker backgrounds. Very pleased with the upgrade. I felt James was excellent to work with always quickly responding to my emails and trying to work out the best possible solution to make me happy.
Did PSA tell you what happened to the other two? Could be just really bad luck, but I’m curious since it’s back to back failures.
James said on the first unit the cable between the IO board and the display had come loose. He said they do have clips which are to help secure it but obviously they came loose. I don’t know about the second unit yet. I emailed James and he is away till January 8 so I’ll follow up when he returns.

It seems like a bit of RTV silicone or a small cable tie applied during assembly would prevent a lot of those “came unplugged during shipping” issues. Especially with the state of consumer shipping choices today.
Silicone in board assembly soldering areas can be a major issue. If used later in processes strict board cleaning processes and testing for silicone residue is a must to ensure reliability of class products. If I had a commercial product house for consumer goods I would try to avoiding it use. .
